Clinical Audit

Bespoke searches can be requested to enable easy monitoring of a local enhanced scheme by commissioning organisations and anonymised outputs can be made available centrally for further analysis. Using the MSDi Practice Comparison application practices can be grouped and compared against any of the pre-authored or commissioned bespoke searches.

MSD Informatics are currently working with a number of CCGs and Local Authorities who use this module to help develop and deliver Enhances Services across a range of National & Local priority areas including Cancer COPD, NHS Health Checks, Diabetes, Mental Health, End of Life and many more.

Medicines Manager

It covers all of the NPC Key Therapeutic Topics but also allows searching for drugs by BNF Grouping, Drug Name or by Patient Name.

The Patient List view includes a wide range of information looking at what conditions the patient is coded as having as well as a range of key clinical recordings such as Blood Pressure, Cholesterol Level, eGFR, FEV1 etc. The software will display graphically the medications which a patient is taking or has been prescribed in the past. It will also overlay investigations and diagnosis to build up a detailed visual management picture linking the initiation and continued prescribing of a drug with the expected improvement in the relevant clinical marker.This allows the reviewer to judge possible compliance with the medication, the appropriateness of drug titration and the effectiveness of each medication.

Some of the searches within Clinical Audit look at patients not currently achieving NICE targets. These patients can be imported straight in to Medicines Management and their current and historical medications can be reviewed and compared with their investigations data. This will allow medicines optimisation and support high quality prescribing within the practice.

Long Term Conditions

Our innovative LtC module is a risk optimisation tool designed to help clinicians plan clinics with a holistic view of their patients and has been highlighted as a best practice example tool for CCGs by NHS England.

The module applies risk profiling to a patient cohort in an intuitive format designed to support the primary care healthcare professional. It includes a fully bespoke display of patients with multiple long term conditions further risk stratified into red, amber, green (RAG) cohorts. Clinicians are able to click on any patient cohort figure to view indicators that are uncontrolled and require attention for specific patients.

Over time, primary care teams can monitor how risk of their patient population shifts from high to low as a measure of a successful LTC strategy. Additional functionality include viewing patients by interaction count to highlight patient cohorts that disproportionally use practice resources and a RAG tracker to allow practices to measure the impact that a local strategy has on patient risk over time.

Quality Manager

Quality Manager consists of:

  • Data Quality Modules, and
  • QoF Maximiser (QMax)

Data Quality Modules ensure that the data extracted from a clinical system does not contain data anomalies and that the data is consistent, complete and error free. The Data Quality Module consists of:

  1. Data Validation.
  2. A list of inconsistencies found during the last audit run.
  3. A reporting tool, which displays the list of errors.

The module extracts data on a monthly basis and checks for errors automatically. These checks include:

  • Patient and medical records have the correct number of fields
  • Event Read Codes are valid
  • Demographic Information
  • Registration data anomalies
  • Invalid Registration Date
  • NHS Number inaccuracies/ duplications
  • Invalid Event Dates

Our QOF Maximiser tool is tailored to search for potential errors that specifically affect the practice QoF targets – this ensures that GP are not unknowingly penalised for any input errors that could have occurred throughout the year.

Health Check

MSDi work with local authorities to provide their data requirements to help support this and other enhanced service programmes.

The NHS Health Check programme aims to help prevent heart disease, stroke, diabetes, kidney disease and certain types of dementia. Everyone between the ages of 40 and 74, who has not already been diagnosed with one of these conditions or have certain risk factors, should be invited (once every five years) to have a check to assess their risk of heart disease, stroke, kidney disease and diabetes and are given support and advice to help them reduce or manage that risk. Our Health Check module supports this programme at practice, community and public health level:

  1. Health Check GP Practice enables practices to…

    • Easily find patients eligible for Health Checks
    • Export patient lists for use as required
    • Sort eligible patients by risk
    • Find out payment achievement to date
    • Confirm how close you are to payment targets

    All patient identifiable data remains at practice level and the software is compatibility with all major GP Clinical System providers. As with all MSDi products, the service comes with full helpdesk support and local training to local practices.

  2. Health Check community enables you to:

    • Accurately capture results during a health check consultation
    • Reduce errors and loss of data vs paper-based alternatives
    • Incorporate captured data into patient clinical record
    • Transfer data safely and securely

    The template is easy to use, and will work on a tablet or laptop making it simpler and easier to complete health checks in community settings.

  3. Health Check Public Health allows you to view health check achievement for each practice across a health economy. Public Health can access to a range of data sets, tracking performance vs target of each practice though Practices Comparison regardless of the local Clinical System(s) in use. Anonymised data at practice level can be viewed then exported and send for NHS England reporting.

Practice Comparison

Compare performance across GP Practices with this web-based tool.

Practice Comparison, a web-based application allows comparison of individual practices or clusters against any of the pre-authored or bespoke searches provided with the software. Pre-authored dashboard reports are available for a range of areas including ethnicity reporting, flu reporting and CVD risk stratification. Individual reports to meet the specific reporting requirements can be written for the NHS Organisation.

The Practice Comparison application for PCOs has been designed to allow the analysis and comparison of anonymised practice data across the PCO. The practices data can be automatically sent using the Clinical Manager application to a central repository at the PCO where it is merged into a central database.

The software is compatible with all major primary care clinical systems. Over a period of time historical data can be accumulated within the application so that future trends can be predicted and the data can be exported to other external applications for further analysis if required.


Clinical Support

CS, Best for Clinician, Best for Patient: Look at the Benefits:

Currently CS is used to support the NHS Health Check Programme, long term condition initiatives, drug-misuse programmes and health determinants/lifestyle programmes as just some examples.

Example of use: If a patient has 3 Long Term Conditions COPD, Heart Failure and Diabetes, then CS creates one template with all the COPD, Heart Failure and Diabetes patient information on one screen. This prevents the user having to complete multiple forms during a co-morbidity clinic and facilitates an integrated approach for the patient consultation. Templates can be split if that is preferred.   1. Patient Centred, ‘One Patient - One Screen’: Using the dynamic functionality each template is tailored to the individual patient. Depending on the patient’s disease status or other factors a personalised template is produced. Patient data can then be presented in electronic or printed format outlining information such as self-management, test results and care pathways.
  2. Bespoke templates to fit local needs: The Template Designer tool allows you to design templates for your own Primary Care Organisation with support from the MSDi team if required. CS Templates can be designed to include decision trees, pathways, data fields, time constraints, READ codes, medications, textual prompts, pictures, links to documents/websites etc. Several templates can be designed to form a complete care pathway or amalgamated to provide all the information on one screen.
  3. Convenience of Single Data Entry: CS displays data from the GP Clinical System with data filed in CS saved back to the patient record, so you only need to enter the data once. Data is shared between templates, for example if a nurse captures a blood pressure measurement in the diabetes template, that blood pressure is also displayed in the CHD template.
  4. Convenience of Automated Updates: CS templates can be distributed centrally and downloaded automatically ensuring the templates in use are always the latest available.
  5. Keep up to date with key patient events: A RED-AMBER-GREEN warning system is applied for out of date items, indicating for example that a yearly cholesterol measurement is due, or out of date.
  6. Better informed decisions - view clinical trends: CS has the ability to show previous data items in a drop down list removing the requirement to look through a plethora of screens.
  7. Ease of access - Patient Medication: The medication of a patient is displayed and the drug dictionary accessed without leaving CS
  8. Ease of access - Risk Tools & Direct Links: A CHD Risk Calculator is integrated within the templates. Documents, websites and other tools are linked within CS
